The Petroleum Safety Authority Norway (PSA) has carried out an audit of Equinor and its application of experience from the start-up and operation of Johan Sverdrup phase 2. The audit was conducted from 15 to 23 August 2023.
The objective of the audit was to carry out an audit of Equinor's application of experience from the start-up and operation of Johan Sverdrup phase 2 for learning purposes, so as to reduce the probability of failure, hazard and accident situations linked to HC leaks.
Three non-conformities were identified, concerning:
Deficient follow-up of the alarm systems
Deficient notification or reporting of incidents
Shortages of temporary containers
In addition, five improvement points were identified, concerning:
Collection of maintenance history
Start-up and operation of facilities
Planning the insulation work
